Patient Records, Licensing & Complaint Information

Lordex Spine Institute provides the following information in accordance with Texas House Bill 4224 and Texas Health & Safety Code §181.105. This page explains how patients and consumers may:

  1. Request health care records from our office;
  2. Contact the licensing or disciplinary authority for Texas chiropractors; and
  3. File a consumer complaint related to protected health information, records access, or chiropractic services.

This information is provided for patient access and transparency. It is not legal advice, medical advice, or a substitute for speaking with the appropriate agency.


1. How to Request Your Health Care Records

Patients may request copies of their health care records from Lordex Spine Institute.

Step 1: Prepare your request

Please include the following information so we can identify the correct records and verify your identity:

  • Full legal name
  • Date of birth
  • Phone number or email address
  • Approximate dates of service or date range requested
  • A description of the records requested, such as office notes, exam findings, imaging reports, billing records, or all available records
  • Your preferred delivery method, such as pickup, mail, secure electronic delivery, or release to another provider
  • Signature and date

If you are requesting that records be sent to another provider, attorney, insurance company, family member, or other third party, we may require a signed authorization before records can be released.

Please do not submit symptoms, diagnoses, Social Security numbers, detailed medical history, or other sensitive health information through a general website contact form.

Patients may also ask the front desk for assistance with a records request form during normal business hours.

Step 3: Identity verification

For patient privacy and security, we may ask you to verify your identity before records are released. This may include confirming contact information, presenting a valid photo ID, or providing a properly signed authorization.

Step 4: Processing your request

Lordex Spine Institute processes record requests in accordance with applicable Texas and federal law. When Texas electronic health record access timelines apply, records are generally provided no later than the 15th business day after a written request is received, unless a lawful exception applies.

A reasonable, cost-based fee may apply for copying, mailing, or preparing records when permitted by law. We do not charge a fee simply to request records.


2. How to Contact the Licensing or Disciplinary Authority

Texas chiropractors are licensed and regulated by the Texas Board of Chiropractic Examiners.

3. How to File a Consumer Complaint

Consumers may file a complaint with the appropriate state or federal agency depending on the nature of the concern.

Option A: File a complaint with the Texas Board of Chiropractic Examiners

For complaints involving a Texas chiropractor, chiropractic care, professional conduct, or chiropractic records, you may file a complaint with the Texas Board of Chiropractic Examiners.

Steps:

  1. Visit the Texas Board of Chiropractic Examiners website or the TBCE Complaint Portal.
  2. Complete the Board’s complaint form in writing.
  3. Include your name and contact information, the name of the provider or clinic involved, the date or approximate date of the issue, and a clear description of the concern.
  4. Attach copies of any relevant documents if available. Do not send original documents unless the Board instructs you to do so.
  5. Submit the complaint online, by email, fax, or mail as directed by the Board.

The Texas Attorney General’s health care agency list states that complaints to the Texas Board of Chiropractic Examiners must be in writing using the Board’s complaint form and may be submitted online or by email, fax, or postal delivery.

Option B: File a patient privacy or protected health information complaint

If your complaint involves protected health information, medical privacy, or a possible violation of the Texas Medical Records Privacy Act or HIPAA, you may file a complaint with one or more of the following:

  • The Texas agency that regulates the provider or business involved;
  • The Texas Attorney General’s Consumer Protection Division; or
  • The U.S. Department of Health and Human Services, Office for Civil Rights.

Steps:

  1. Visit the Texas Attorney General Patient Privacy page or Consumer Protection Division complaint page.
  2. Select the option to file a consumer complaint.
  3. Provide a clear statement of the issue, including important dates, names, and supporting details.
  4. Attach copies of relevant documents if requested or helpful.
  5. Keep a copy of your submission for your records.
